SUPPORTED EMPLOYMENT PROGRAM - JOB COACH


JOB SUMMARY: The Job Coaches should provide services for the Supported Employment Program by performing duties utilizing appropriate and effective methods as directed. Such duties include, but are not limited to: on-site job coaching, vocational counseling, pre- employment planning and assessment of client's work potential.

REPORTS TO: SEP Manager

JOB RESPONSIBILITIES:

• Supervise client on the work site per assigned schedule
• Complete appropriate Department of Rehabilitation and VAC required forms
• Coordinate client start dates with SEP Manager, Job Developer and employers to insure a smooth transition to the job site and that adequate support/staffing is in place
• Train the client according to basic employer requirements including reporting procedures, hours, rate of pay, employer expectations, rules, and benefits.
• Confer with the employer and client during the first 90 days of employment at the direction of the Job Developer as necessary to discuss conflict resolution, advocacy for promotion and overall satisfaction of the client, etc., to ensure the long term success of the client's placement.
• Implement pre-employment classes for clients identified as SEP candidates as required
• Complete: Job Coaching notes each day for each client, monthly tally for Monthly Job Coach Report, rough draft of Client Monthly Progress Report, evaluations of efficiency and performance of the SEP clients as needed
• Distribute Monthly Employee Survey to the employer and the Monthly Client Job Survey to each client at the end of each month
• Maintain a system of reports for effective client evaluation while maintaining strict client confidentiality
• Encourage clients to develop socialization, work ethics including attendance and punctuality, appropriate dress, personal hygiene, etc.
• Complete other related duties as assigned

JOB QUALIFICATIONS:
• High School Diploma or G.E.D.
• Excellent time management, written and oral communication and computer (Word, Excel) skills.
• Work nights or weekends if required (schedules are determined by the clients working schedule).
• Interest in working with the disabled community.
• 6 months experience working with autistic individuals or people with developmental disabilities preferred.

CERTIFICATES, LICENSES AND REGISTRATIONS:
• Valid Driver's License
• Reliable and safe vehicle with proof of insurance
