SUPPORTED LIVING SERVICES - DIRECT CARE STAFF

JOB SUMMARY: The Direct Care Staff should provide assistance, support and encouragement to clients according to their Individual Program Plan. This might include but is not limited to: assisting with daily living tasks, transportation, regular and emergency medical and dental appointments and other related duties.

REPORTS TO: Supported Living Services Administrator

JOB RESPONSIBILITIES:

• Establish and maintain appropriate interaction with clients, parents and/or legal guardians of clients and Kern Regional Center's Service Coordinator.
• Attend any and all meetings which are assigned.
• Minimize risk of endangerment and health to the client in the client's home and in the community.
• Assist, support and encourage the client with daily living skills according to the individual's IPP.
• Respond to client emergencies by VAC and SLS protocol (stated in the IPP).
• Maintain client's: daily documentation, medications sheet, daily activity calendar, appointment calendar, etc.
• Refill client's prescriptions if needed.
• Any other duties that are required to comply with Valley Achievement Center's Supported Living Services contractual agreement.

JOB QUALIFICATIONS:
• High School Diploma or G.E.D.
• At least 6 months (1 year preferred) providing direct care services to adult consumers (18-65 yrs.) similar to Kern Regional Center clients.
• Very good written, verbal and time management skills.

CERTIFICATES, LICENSES AND REGISTRATIONS:
• Valid Driver's License
• Current and valid Infant, Child and Adult CPR and First Aid Certification.
• Reliable and safe vehicle with proof of insurance.
